Based in Toronto, Ontario, our client is one of the most established and comprehensive community health organizations in the province. They operate three Community Health Centres that offer Primary Health Care Services.

Primary Care Director

Purpose:    Reporting to the Vice President Community Health & Chief Professional Practice,
The Primary Care Director will lead and support the CHC interprofessional teams in the development and delivery of Primary Care health services to the community.

  • Capacity to develop and maintain professional links with partner agencies and key program stakeholders
  • Demonstrated knowledge and commitment to the principles and practice of health promotion and education and as well as the social determinants of health
  • Demonstrated experience in program development and evaluation, project management and LEAN
  • Strong organizational skills to manage competing priorities and balance a multitude of tasks
  • Demonstrated leadership, team development and facilitation skills coupled with superior interpersonal, relationship and communication skills
  • Capacity to develop and maintain professional links with other agencies

Educational and/or Professional Qualifications:

  • Current registration and good standing with one of the Regulated Health Discipline Colleges of Ontario -  Professional liability insurance is required
  • Bachelor’s Degree, Master Degree preferred,  related to your health discipline or Health Administration required

Level of Experience:

  • Three to five years management experience
  • Experience working in a primary care setting and an understanding of quality improvement and client safety is required
  • Experience in program development, implementation, monitoring, evaluation & staffing and scheduling is required
  • Experience in budgets/statistical reporting, variance analysis and performance measures is required
  • Current driver’ license and access to a car
