The Nurse Practitioner (NP) is a nurse specialist in Primary Health Care who works with a primary focus of persons marginalized by social location including homelessness, poverty, sexual identity, gender identity, and residency status. The NP will have advanced knowledge and decision-making skills, built on basic nursing preparation and on advanced academic preparation and/or clinical experience which integrate health related theories and research.

Duties include: Working closely within a multidisciplinary team to provide care throughout the client’s life span through a variety of methods including: comprehensive nursing assessments, health promotion and education, outreach to marginalized individuals and groups, advocacy, and community development.



  • A generalist nurse practitioner holding an Extended Class Certificate of Registration with the College of Nurses of Ontario.


  • At least three years experience working with persons marginalized by social location including homelessness, poverty, sexual identity, gender identity, residency status
  • Experience obtained in either primary care or a community setting is preferred
  • Experience with communities facing barriers to accessing health care due to social isolation, homelessness, mental health and/or substance use issues.
  • Knowledge of available resources to support the determinants of health.

Skills and Abilities:

  • Demonstrated knowledge of clinical theories and practices, pharmacology and the determinants of health
  • Demonstrates an understanding of working within an anti-oppression framework
  • Excellent verbal and written communication skills; ability to adapt communication style to audience
  • Ability to effectively organize, time, plan, research and evaluate tasks and projects
  • Developed conflict management and problem solving skills
  • Works effectively independently and as a member of a multi-disciplinary team
  • Commitment to a healthy and safe workplace for self and others by taking all reasonable precautions and working in compliance with SHC related policies, health and safety legislation and best practices.

Conditions of Employment:

  • Employment is contingent upon the successful completion of a Police Records Check and Vulnerable Sector Check.
  • Understand and maintain compliance with the Occupational Health and Safety Act (OHSA), Personal Health Information Protection Act (PHIPA), Accessibility for Ontarians with Disabilities Act (AODA), the Ontario Human Rights Code and other relevant legislation.

Working Conditions:

  • Standard work week is 37.5 hours. However, this position requires the ability to work flexible shifts including regular days, evenings and weekends.
  • The Nurse Practitioner will be required to work offsite e.g. Mobile Health Bus, home care visits, and satellite locations.
