Family Resources Worker

Contract: Approximately 8 months

An exciting opportunity exists in our Health Department for a Family

Resource Worker. As part of a multidisciplinary team, you will be

responsible for providing support and information to developmentally

challenged children and youths, their families and caregivers. Duties will include: identifying client needs and developing individual client-centered, goal oriented programs with each client; assisting clients, their families, and caregivers to make effective use of the community services necessary to meet identified needs by referring to appropriate agencies, coordinating service delivery, and liaising between the family and the agencies involved; advocating with or on behalf of the client/family, at their request, to acquire resources required to meet their needs; and empowering the client, their families and caregivers to independently and effectively manage their needs.

Qualifications include: * a degree or diploma in social work or a

health science field, with an emphasis on developmental disabilities *

minimum three years experience working with individuals who have a

developmental disability and their families or caregivers * knowledge of Child and Family Services Act, Developmental Services Act, and community resources * excellent communication (both verbal and written) skills * demonstrated planning and assessment skills and ability to motivate and deal effectively with people.
