As an integral member of a multidisciplinary team, you will be involved in supporting individuals and families affected by tuberculosis including the delivery of directly observed therapy. You will also document and record information according to established communicable disease policies/practices, identify social, cultural and economic barriers that may prevent clients from completing the treatment plan and facilitate referrals to appropriate community resources in order to enhance the completion of TB follow up plans.
Success requires: * post secondary education in a health or social services field, with a strong emphasis on community work * minimum three years experience as a health outreach worker * current knowledge of tuberculosis, social/health issues, and of the community served * excellent communication skills along with strong facilitation and family assessment skills * experience working with diverse communities * fluency in other languages would be considered an asset * good quantitative and qualitative research skills * access to own transportation and willingness to work some evenings and weekends.
Salary range is $36,358 - $49,195 (subject to review).
