Public Health Nurse/Heart Health Co-Ordinator

The Hastings and Prince Edward Counties Health Unit requires a Public Health Nurse/Heart Health Co-Ordinator 1.0 FTE (Permanent Full Time)

Reporting to the Director, Healthy Lifestyles and to the Heart Health Steering Committee the incumbent will participate in the Heart Health Project - a comprehensive, 5 year, community based health promotion project funded by the Ontario Ministry of Health and in-kind contributions from the local community partners. This project addresses the three modifiable risk factors for heart disease: smoking, unhealthy eating and physical inactivity. The Heart Health Co-Ordinator will participate as a member of the Steering Committee in the development, co- ordination, implementation and evaluation of the heart health programs. After March 31, 2003, the incumbent will participate as a member of an inter-disciplinary team in planning, development, implementation, coordination and evaluation of the Chronic Disease Prevention program to meet the requirements of the Mandatory Health Programs and Services Guidelines, 1997.


A current Certificate of Registration with the College of Nurses of Ontario. A Baccalaureate degree in Nursing which includes Public Health preparation. Knowledge of/experience in group facilitation. Excellent organizational skills. Excellent communication skills (oral, written & media). Self directed, demonstrated leadership skills and excellent inter-personal skills. Valid Ontario Driver's licence and vehicle. Basic computer skills (WordPerfect 6.0) and use of Internet. Flexible working hours are required.
