Community Health Worker, The Four Villages Community Health Centre

1.0 FTE

Permanent full time, 35 hours per week

The Four Villages Community Health Centre, located in the Bloor-Keele
area of west Toronto, is a health centre funded by the Ontario Ministry
of Health and Long-Term Care.  Our mandate is to promote increased
access to health and wellness for our communities within the context of
the broader determinants of health.  Our wide range of programs as
well as our clinical, general health and social services, are primarily
aimed at seniors, families with young children and adolescents, and
newcomers. Staff collaborate in an interdisciplinary team environment;
the focus is on the client and capacity building.  We are
currently seeking an energetic Community Health Worker who will become
a member of our interdisciplinary team.

The successful candidate will participate in the development,
implementation and evaluation of health promotion programs which foster
self-help, mutual aid and capacity building and address needs of
community, especially seniors and other priority populations who face
barriers in accessing health and wellness.  As a member of the
interdisciplinary primary care team, you will work within the framework
of the determinants of health to engage the community and plan
effective responses to identified health needs.  

The successful candidate will possess the following qualifications:

  • Degree/diploma in a health or social science discipline from a
    recognized university or a combination of some post-secondary education
    and significant experience in health promotion and/or community
    development;
  • Three to five years relevant experience in a community setting;
  • Extensive experience in community development, outreach, organizing, and/or health promotion;
  • Experience working with seniors and other vulnerable populations in a community setting;
  • Extensive experience in community based program planning, implementation, monitoring and evaluation;
  • Excellent group facilitation skills;
  • Demonstrated ability, commitment to and knowledge of community health care;
  • Demonstrated ability to work in an interdisciplinary team
    environment, as well as ability to take initiative and work
    independently;
  • Success in partnership development and proposal writing;
  • Excellent interpersonal skills with clients in a culturally diverse community, and with colleagues;
  • Excellent communication, decision-making, problem-solving, conflict management and time management skills;
  • Proficiency in the use of computers and appropriate software applications;
  • Second language, relevant to catchment area populations, is an asset.

Hours of work: 35 hours per week, requirement to work flexible hours
Salary: Based on an annual salary range of $39,923-48,100 plus excellent benefits
